So what is it doing in there? More than you’ve been told. The Global Consensus Position Statement on the use of testosterone therapy for women, backed by an analysis of dozens of randomized controlled trials, concluded that testosterone at physiologic doses, meaning doses that restore your blood levels to the normal premenopausal range and no higher, is an evidence-based treatment for one condition: hypoactive sexual desire disorder in postmenopausal women. In plain language, that’s low sexual desire that causes you real distress. Not a low number on a lab report. Not a libido that differs from some imaginary standard. A loss of desire that you feel and you mind.

For that problem, the trial data is genuinely good. Women treated with physiologic testosterone reported meaningful improvements in desire, arousal, orgasm, pleasure, and overall sexual satisfaction, along with less of the distress that brought them in. That’s the strongest evidence base testosterone has in women, and it’s strong enough that the world’s menopause experts put their names on it.

What about everything else: the energy, the mood, the mental sharpness so many women hope it will restore? This is where honesty matters. Plenty of women on testosterone report feeling brighter, more driven, more themselves, and small studies point in encouraging directions. But the rigorous trial evidence isn’t there yet, and the same consensus statement declined to recommend testosterone for mood, energy, cognition, bone, or muscle as standalone reasons to treat. That doesn’t mean those benefits aren’t real. It means science hasn’t finished the work of proving them, in part because research into women’s hormones has been underfunded for decades. A trustworthy clinician will tell you exactly that: possible, plausible, not yet proven. Anyone who promises testosterone will fix everything is selling something.

The double standard

Now for the part that should make you a little angry, because it makes us a little angry. In the United States, there are more than thirty FDA-approved testosterone products for men. Low-T clinics advertise on billboards and sports radio. A man who mentions fading energy to his doctor will very likely leave with a lab order and a follow-up plan.

For women? Zero. Not one FDA-approved testosterone product exists for women in this country, despite the evidence you just read about, and despite the fact that women’s bodies make and use this hormone every day of their lives. Products designed for women have been developed and submitted over the years; none made it through, in part because regulators demanded long-term safety data that male products were never required to produce. The result is a strange limbo: the treatment is legitimate, the evidence for the right indication is solid, and yet every prescription for a woman is off-label, either a small, carefully measured fraction of a product dosed for men, or a version prepared by a compounding pharmacy under a clinician’s direction.

Off-label prescribing is common, legal, and often exactly the right medicine. But sit with the asymmetry for a moment. The same hormone, in the same country: for him, a menu of approved options and a clinic in every shopping center; for her, a workaround, if she can find one of the relatively few clinicians comfortable prescribing it at all. This is the care gap in miniature, the same one that runs through all of menopause medicine. It’s not that answers don’t exist. It’s that nobody built the system to deliver them to you. Myths and cautions

It won’t make you masculine. High doses are another story.

Let’s retire the fear that keeps most women from even asking the question: will testosterone make me look or sound like a man? At physiologic doses, no. When your levels are restored to the normal female range and kept there, you are simply back to a version of your own chemistry, not borrowing someone else’s. The masculinizing changes people worry about, a deepened voice, coarse facial hair, other irreversible shifts, are effects of supraphysiologic dosing: levels pushed far above anything a woman’s body would naturally produce, sustained for a long time. That’s not a risk of careful treatment. That’s a consequence of careless treatment.

The side effects that do show up at appropriate doses are milder and more manageable: some women notice a bit of acne or increased hair growth, often at the application site. These are dose-related, which means they’re also dose-fixable: lower the dose, and they typically settle. This is exactly why good care involves checking your blood levels before you start, confirming they stay within the normal premenopausal range once you’re on treatment, and keeping an eye on how you actually feel. Monitoring isn’t bureaucracy. It’s the difference between medicine and guesswork.

Be cautious with pellets and “optimization” dosing.

Some clinics aggressively market testosterone pellets, implanted under the skin, at doses that push women’s levels far above the normal female range, promising energy, youth, and everything in between. Two problems: supraphysiologic levels are where the real side effects live, and once a pellet is implanted, it cannot be adjusted or removed; you’re committed for months. Most menopause specialists prefer adjustable transdermal forms, like a daily cream or gel, that can be tuned to your labs and your response. The takeaway isn’t that testosterone is dangerous. Used at the right dose, with monitoring, it has a reassuring safety record in trials. The takeaway is that the dose and the oversight are the whole game. Respect for this hormone and enthusiasm for it are not opposites; they’re partners.
