Why are your ears ringing?
A high-pitched ring, a low buzz, a soft hiss, arriving out of nowhere and following you into the quiet. Some women describe stuffy or “underwater” ears too. It’s one of the stranger, less-talked-about parts of perimenopause. You’re not imagining it, and you’re not alone.

Estrogen has a hand in how your auditory system works: blood flow to the ear, nerve signaling, the way your brain processes sound. As estrogen swings and falls, some women notice new ringing, buzzing, or a stuffy, underwater feeling. And because poor sleep and stress often show up in this same stage, they can turn the volume up on tinnitus that’s already there. New or changing ear symptoms should always be checked out first, but for many women, hormones are part of the story.
Estrogen reaches all the way into your ears
It surprises most women to learn that the ear is a hormonally active place. The inner ear, the coiled, fluid-filled cochlea and the fine hair cells inside it that turn vibration into the signals your brain reads as sound, is dotted with estrogen receptors, and so are the nerve pathways that carry those signals upward. Estrogen isn’t a bystander in hearing; it’s one of the quiet regulators that keeps this system tuned. When your levels hold steady, the machinery hums along and you never think about it. That’s the whole point of a system working well: it disappears.
One of the biggest things estrogen looks after is blood flow. The inner ear is astonishingly hungry for a steady, well-oxygenated supply: its hair cells are among the most metabolically demanding in the body, and they’re fed by some of the tiniest, most delicate vessels you have. Estrogen helps keep those vessels relaxed and open and the circulation smooth. In perimenopause, estrogen doesn’t ease down in a gentle line; it lurches, high one week, low the next. When it dips, the flow to those exquisitely sensitive cells can waver, and cells that are used to a constant supply notice the change. Sometimes that shows up as a new ring or hiss that seems to arrive from nowhere.
There’s a fluid story here too, and it’s the one behind that stuffy, “underwater” feeling. The inner ear runs on a finely balanced system of fluids held at precise volume and pressure, the same system that governs both hearing and your sense of balance. Estrogen and its partner hormones have a hand in how the body manages fluid and pressure, so when they swing, that delicate internal balance can shift with them. The result is a sensation of fullness or muffling, as if you need to pop your ears or you’re hearing the world through water, even when nothing is actually blocking them.
And then there’s your brain, which is really the conductor of the whole thing. When the signal coming up from the ear changes, even subtly, the brain tries to fill the gap, and one way it does that is by generating sound of its own. That’s a large part of what tinnitus is: not a noise in the room, but the brain’s response to a shifting input. Layer on the broken sleep, the higher stress, and the surges of cortisol that so often ride along with this stage, and a nervous system that’s already on alert will turn that internal sound up and make it harder to tune out. This is why ringing ears rarely travel alone: they tend to show up in the same season as hot flashes, disrupted sleep, and brain fog, all threads of one hormonal shift rather than separate, unrelated problems. The sound is real and physical. It isn’t “all in your head,” and it doesn’t mean you’re imagining things.
Ringing ears can show up in perimenopause, but tinnitus has many possible causes, plenty of them with nothing to do with hormones. Getting it evaluated isn’t overreacting; it’s the right first step.
See a doctor or an ENT about new or changing ear symptoms, and seek prompt evaluation if you have sudden hearing loss, ringing in only one ear, or ringing that comes with dizziness or ear pain. Those signs need to be checked without delay. Ruling out an ear, nerve, or other medical cause comes before assuming it’s hormonal.
Here’s the pattern we see often: a woman mentions the ringing, gets told it’s just stress or aging, and leaves without an answer. If your ears have been examined, your hearing checked, and the sound keeps coming alongside other changes like broken sleep, hot flashes, or brain fog, that’s exactly when it’s worth asking about your hormones.
Once other causes have been ruled out, the aim shifts from bracing against the sound to steadying the things that feed it. There’s rarely one switch that turns tinnitus off. More often it eases because several small things line up: the hormonal swings settle, the nervous system calms, sleep gets deeper, and the brain slowly stops treating the sound as something to guard against. Look at the hormones underneath. If the ringing arrived alongside other signs of the shift, the swings themselves are worth putting on the table with a specialist. Give the quiet something to hold. Tinnitus almost always feels loudest in silence, because in silence there’s nothing else for your attention to land on. Many women find real relief in gentle background sound: a fan, soft music, a rainfall or white-noise track at night, a window cracked to the world outside. The point isn’t to drown the ringing out but to give your brain a wider soundscape, so the internal sound stops being the only thing in the room. Bedtime, when the house goes still, is often when this matters most.
Protect the hearing you have. Ears already working harder don’t need extra strain. Easing back on very loud environments, and using simple ear protection at concerts, with power tools, or anywhere the noise makes you raise your voice, spares the same delicate cells the hormonal shift is already taxing. It’s a quiet, preventive habit rather than a cure, but it keeps you from adding to the load.
Treat the sleep and the stress as part of the problem, not a side issue. This is where the volume knob really lives. A tired, over-alert nervous system amplifies the ringing; a rested, settled one lets it recede. Anything that genuinely lowers the background hum of stress, whether slow breathing, a wind-down routine, time outdoors, movement you enjoy, or protecting sleep however you can, tends to make the tinnitus feel smaller, even when the underlying signal hasn’t changed. It’s not that you’re relaxing the sound away; it’s that a calmer brain stops holding it in the spotlight.
Notice your own triggers. Some women find that caffeine, alcohol, salty stretches, or a badly slept night nudge the ringing up for a while. None of these is a universal culprit, and there’s no need to strip your life bare hunting for one, but paying loose attention to what tends to precede a louder day can hand you a small measure of control, and control is often what’s been missing.
Above all: don’t dismiss it, and don’t let anyone wave it away. The ringing is real, it’s worth raising, and naming it as part of the hormonal picture is often the first step toward turning the volume back down.
Why it seems to get louder the more you listen
Here’s something almost every woman with tinnitus notices, and almost no one gets explained: the ringing seems to swell in the quiet, at night, and precisely when you turn your attention toward it. That isn’t your imagination playing tricks. It’s the way the brain sets the volume, and understanding it can take some of the fear out of the sound.
Your brain is constantly deciding what deserves your attention and what can safely fade into the background. Most of the sounds around you never reach your awareness because your brain has quietly filed them as unimportant. Tinnitus is different, because the brain hasn’t decided yet whether this new sound is a threat. So it keeps flagging it, pulling it forward, checking it, holding it in the spotlight. The more alarmed you feel about the ringing, the more the brain treats it as something worth watching, and the louder and more insistent it becomes. It’s a loop: the sound raises your alarm, and your alarm turns up the sound.
This is the hopeful part. Because the volume lives partly in attention and not only in the ear, it isn’t fixed. Brains are remarkably good at learning to push a steady, harmless signal back into the background, the same way you stop hearing a clock ticking or a fridge humming a few minutes after you walk into a room. It usually happens gradually, and it happens faster when the sound stops feeling like a threat and the nervous system settles. Knowing that the ringing is a real, explainable response to a hormonal shift, rather than a sign that something is going wrong, is itself part of how the brain learns to let it fade.
